Although there are a number of specific steps we follow and techniques we use when conducting an independent investigation, our Investigations service is centered around five (5) simple steps:

• Establish the Investigation Scope;

• Conduct Interviews and Research;

• Determine the Root Cause(s);

• Formulate Practical Recommendations; and

• Issue Investigation Report.

The end result of our investigation is an honest, straight-forward written report defining the original root cause(s) of the problem, the sequence of events that lead up to it (in flowchart format) and a number of recommendations targeted towards:

• Correcting the existing situation (reactive); and

• Preventing its reoccurrence in the future (proactive).
